Application for Employment
(P-105S)
This easy-to-complete application form contains
the type of questions that will give you better
insight into the applicant’s skills, ability, and
other important background information. It helps
you identify the applicant’s goals, interests, and
dislikes. The applicant’s signature block gives
you general authority to make background checks and
complete other types of tests or screens available
under applicable state or federal law (additional
consent/authorization forms may be required as provided by your
CRA - Consumer Reporting Agency), and contains important
“employment-at-will” language.
Reference Check/Verification
(P-126)
This handy form allows you to properly record information
when making reference checks with previous employers, or
when verifying driver’s license or other license or
certificate information provided by an applicant.
Personnel Memo
(P-141)
If every supervisor has a pad of these easy-to-use forms
available, your personnel files stand a much better chance
of being properly documented! It serves as a multi-purpose
document when recording basic file information such as
changes in rates of pay, address, position, department, etc.
It also can be used to record the issuance of employer
property, payroll advances, or the completion of training
programs and approval of related training expenses. The
reverse side may be used to report and record absences, and
provides documentation of disciplinary actions.
Self-Evaluation
(P-156)
This form uses the latest concept in evaluating employee
commitment to improved levels of customer satisfaction
and to profitability of the organization as a whole. It
enables identification of actions needed for improvement
in each of the three areas as they relate to customer
satisfaction and profitability: Quality of Work, Volume
of Output, and Interpersonal Relationships. When
completed, the form provides an easy-to-discuss review
of an employee’s personal and perceptive evaluation of
his/her own strengths and weaknesses.
Performance Goals
(P-157)
This form does away with the hazards and fallacies of
older “point system” performance evaluation forms.
Used in conjunction with the Self-Evaluation or
separately, it focuses on goals needed for employees
to make a stronger personal contribution to the highest
possible level of customer satisfaction and to
profitability of the organization as a whole. Space is
provided for identification of goals in each of the
three areas: Quality of Work, Volume of Output, and
Interpersonal Relationships. Following each goal, a
description of the actions needed for goal attainment
may be entered. Contains disclaimer regarding any
promise that attainments of goals will guarantee employment.
New Employee Report
(P-160)
This form contains important
information about new employees, assures you that all paperwork
has been properly completed, and records the issuance of employer
property. It includes a check off of discussions covering
personnel policies, compensation, and duties of the position.
Attendance Record
(P-535)
Using a "year-at-a-glance" format, this form allows you to post
and code absences and provides an easy method of tracking
vacation and sick leave eligibility and utilization.

This FREE form makes tracking multiple vacation schedules at once.
You can use one sheet for the whole company, or a different
one for each department.
Separation Report
(P-720)
This form is a must in properly documenting terminations. It
answers future questions regarding charges of discrimination
or unemployment insurance claims. Includes check off for return
of employer property and important group insurance and COBRA actions.
Leave of Absence Request/Approval
(P-800)
This form documents the requirements and conditions of all types
of leaves of absence. It helps you comply with applicable state
and federal laws covering disability, pregnancy, childcare, and
other forms of leaves.
In compliance with regulations controlling the Family and
Medical Leave Act, “Fact Sheet” information must be given to all
employees covered by the act at the time of employment and at
the time a covered leave of absence begins.
